Last week I was pretty surprised to find out not many people on here had not heard of Piper Alpha. It is the worst offshore loss of life in history and changed the way safety is handled day-to-day, in design, and the way that the government controlled working conditions.

167 died in the disaster. It's the 25th anniversary next month and the Step Change in Safety initiative created this video. It's pretty hard to watch but very well done.
