A few years ago, my brother was in an Indian restaurant after a night on the tiles and needed the toilet, as you do. He merrily went about his business, only to find afterwards that the damn thing wouldn't flush.

In his inebriated state he decided to take the cover off the cistern to see what's up (I assume the world wasn't ready to see whatever he'd left in the bowl). However the heavy ceramic cistern cover slipped out of his hands and fell into the toilet bowl, smashing it into pieces and depositing the contents all over the restroom floor.

Quick apologies and hasty exit was made.
