References on job applications

This has probably come up multiple times before and should definitely be something I've learnt by now, but I haven't, so please lend a thought.

On my CV it says references available on request. However, I'm applying for a job where I have to fill in an application form, and it has a space to include two referees. My issue is, I feel one should be my current boss, but I don't really want them ringing him while I'm still sat at my desk in front of him. References aren't usually consulted until late in the process right? Like, after an offer is made? Should I send an email with the form saying please don't contact references until such and such? Seems a bit needy.

Also, one reference is specified as an academic reference. I left uni four years ago. The job doesn't have a particularly academic element. I'm not in touch with any of my old tutors, so chasing one up would be a bit of a hassle. Seems a bit weird, no?
